VTT Brazilian jiujitsu  Waiver and Release of Liability

I, the undersigned, hereby acknowledge and agree to the following terms and conditions in consideration for being allowed to participate in activities, programs, and use the facilities provided by VTT jiujitsu at 1900 north Washington blvd, Sarasota (hereinafter referred to as "the Academy"):

1. Assumption of Risk: I understand that participating in martial arts, including but not limited to Brazilian Jiu-Jitsu and Muay Thai, involves inherent risks of injury. I am aware that these activities may result in personal injury, including but not limited to bruises, sprains, strains, fractures, and more severe injuries. I voluntarily accept and assume all risks, known and unknown, associated with my participation in the Academy’s activities.

2. Release and Waiver: I hereby release, waive, discharge, and covenant not to sue the Academy, its owners, instructors, employees, and any affiliated entities from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in Academy activities.

3. Medical Treatment Authorization: In the event of an injury or medical emergency, I authorize the Academy staff to secure and administer medical treatment deemed necessary. I understand that I am responsible for any medical expenses incurred.

4. Rules and Regulations: I agree to abide by all rules, regulations, and instructions provided by the Academy. Failure to comply may result in my removal from the Academy without a refund.

5. Photography and Video Release: I grant the Academy the right to use photographs and videos of me for promotional and educational purposes without compensation.

6. Membership Terms: I understand that my membership is on a month-to-month basis. If I wish to cancel my membership, I must provide written notice to the Academy at least one (1) week before my next scheduled payment. Failure to provide timely notice may result in continued charges for the following month, for which I am responsible.

7. Acknowledgment of Understanding: I have read this waiver and release of liability, fully understand its terms, and acknowledge that I am signing it voluntarily and without any inducement or assurance of any nature.
